Depends on if you mean surface rust or a rust through hole. I'm assuming that it's surface rust, as a 4x6 hole would render the car undrivable in anything but warm sunny weather. You need to sand the rust off to shiny metal. There are a number of rust inhibitors out on the market, check Eastwood Co's site on the net, they specialize in resto materials and prep the shiny metal before going back with primer and paint.
The whole roof of my gt had surface rust from sitting on blocks outside for 18 years. I used Tal Strip to get off what was left of the paint and then took a drill and a wire brush attachment to it. It got the remaining paint and all of the rust off in about two hours. Next I painted it with a rust inhibiting primer... It will stay that way until I am ready to do the final body work.
If the metal is badly pitted or rusted through you will need new metal patch panels welded in.
